Submission
1. Packet receipt: University of Colorado (CU) Polar Medicine administration receives packet request. PQ packet (forms and instructions) are emailed to recipient.
2. Packet completion: Participants complete required documentation, testing and examinations as identified in the original email.
3. Packet submission: Submission occurs via Epic Chart and My Health (CU Admin creates Epic Chart, Applicant creates MyHealth Connection Chart.)

4. Initial review: Once package is submitted, medical and dental records are reviewed by CU Polar Medicine administration. Missing items are identified and requested.
5. Follow up: Additional documentation and/or missing items are requested by administration.
6. Final Submission: All documentation is submitted. Administration reviews for completeness and forwards for medical and dental reviews.

Process
7. Physician and dental reviews: Packet reviewed by physician (medical and labs) and dentist (dental and x-rays). Identifies medical/dental concerns that may affect qualification based on the criteria in the PQ Guidelines. Additonal information/documentation may also be requested in this step.
8. Initial determination: Participant receives either a Physical Qualification (PQ) or Not Physically Qualified (NPQ) notification in writing.
9. Waiver process: If NPQed, a waiver can be requested (directions are provided in the determination letter.) Once the required information is received it will be forwarded to NSF for final review and determination.
